This clinic offers essential primary care support for patients who do not have access to a family doctor or nurse practitioner on a regular basis.
About the North York Community Care Clinic
The North York Community Care Clinic (NYCCC) connects patients with a nurse practitioner on a temporary basis to ensure they receive necessary care while they wait for a permanent primary care provider (family doctor or nurse practitioner).
This clinic provides essential health care services, including health checks, chronic condition management (for diseases such as diabetes or heart disease), prescriptions, cancer screenings and vaccinations. You will receive care through our partners, Get Well Clinic and Baycrest Hospital.
Eligibility criteria
To access the North York Community Care Clinic, patients cannot have an existing family doctor or nurse practitioner.
Patients must be at least 18 years old and have one or both of the following:

Two (2) or more chronic conditions such as diabetes, arthritis, mental health related concerns, addictions or hypertension
OR
-
At least one (1) of the following conditions:

Chronic obstructive pulmonary disease (COPD)

Congestive heart failure (CHF)

Cancer or other serious illnesses

Patients must also live in North York and be able to travel to the clinic in-person either at:

Baycrest Hospital (3560 Bathurst St, Toronto M6A 2E1)

If you meet the criteria of our clinic, you can complete the online self-referral form or call 416-508-5691 and press extension ‘9’ and ask for the North York Community Care Clinic.
This program is a collaboration between North York Toronto Health Partners Ontario Health Team (NYTHP OHT), Get Well Clinic, Baycrest Hospital and Flemingdon Health Centre, and is funded by the Government of Ontario.
